By using the Freedom From Religion Foundation's (FFRF) Out of the Closet National Video Campaign application, you consent and agree to the following terms. If you do not agree, please do not use this application.

1. I attest that the video I uploaded is a video of me.

2. I fully consent to the use of my name, video footage, photographic image, likeness, statements, quotes, and other personally identifiable information contained in the video, in whole or in part, individually or in conjunction with other images, text, or videos, as part of the FFRF's Out of the Closet National Campaign.

3. I understand that, if approved, my video will, at minimum, post to and appear at FFRF's website (FFRF.org) along with other Out of the Closet submissions, as well as FFRF's Youtube channel.

4. I understand my video may be used for publicity and promotional materials by FFRF including, but not limited to, television advertising, posting on FFRF's website, fundraising materials, Freethought Today, or any other materials (printed or electronic) distributed by FFRF.

5. I understand that, by submitting my video for consideration in FFRF's Out of the Closet National Campaign, I waive all rights of privacy and compensation in connection with the use of my name, video, photographic image, likeness, statements, and other personally identifiable information contained in the video.

6. I agree not to post any material which is knowingly false, libelous and/or defamatory, inaccurate, abusive, obscene, vulgar, profane, slanderous, hateful, threatening, harassing, sexually-oriented, invasive of a person's privacy or any other material that may violate any laws be it of the United States, my country, or International Law.

7. I agree not to post any copyrighted material unless the copyright is owned by me or I have received express permission from the owner.

8. I understand and agree that FFRF retains the right to approve or disapprove of any video I submit under this application. I understand that FFRF will only approve videos primarily featuring actual human subjects (i.e., no videos exclusively of pets, cartoon submissions, scenery, etc.).

9. In order to ensure that the videos submitted for FFRF's Out of the Closet National Campaign meet the approved terms, FFRF maintains the right to edit the video for size and/or content. I hereby waive any right to approve or inspect the final copy that FFRF may use for the Out of the Closet National Campaign.

10. I understand that there are inherent risks associated with the publication of my image of personal likeness in digital recordings, video, images, and/or text posted on the internet or any other medium. I agree to assume these risks, whether known or unknown to me, of permitting publication of my image of personal likeness in the form of a photograph, still or moving image, and/or audio on the internet, in publications, or in videos both now and in the future.

11. I hereby forever release and discharge FFRF and its legal representatives from any and all claims, actions, demands, and liability relating to, arising out of, or in connection with the use of videos or images submitted for FFRF's Out of the Closet National Campaign including, without limitation, any and all claims for invasion of privacy and libel.

12. If my video submission includes the image of personal likeness of any other individual(s) (e.g., my spouse, my child(ren), etc.), I attest that I have permission and/or legal authority to sign on behalf of and thereby authorize the application of the terms of this waiver to the individual(s) included.

13. By submitting a video to the campaign, I certify that I am an adult over the age of 18 and that I am free and able to give consent.

 

Terms of Use and Consent Form For Minors Ages 13-17

By submitting a video for the Freedom From Religion Foundation’s (FFRF) Out of the Closet National Ad Campaign, you consent and agree to the following terms.  If you do not agree, please do not submit or allow your child to submit a video.

 I attest that I am the parent or legal guardian of the child between the ages of 13 and 17 who seeks to submit a video for FFRF's Out of the Closet Campaign.

I certify that I have given my child or dependent (hereinafter “child”) permission to take part in FFRF’s Out of the Closet National Campaign.

I attest that the video uploaded is a recording of my child.

I fully consent to the use of my child's video representation, photographic image, likeness, voice, actions, statements, and other personally identifiable information contained in the video, in whole or in part, individually or in conjunction with other images, text, or videos, as part of the FFRF’s Out of the Closet National Campaign.  

I understand that, if approved, my child's video will, at minimum, post to and appear at FFRF's website (FFRF.org) along with other Out of the Closet Videos.
